Aboriginal Youth In Transition
Aboriginal Youth In Transition Worker serves Aboriginal youth 16 -24 years old who are or have ever been a Crown Ward or involved with a CAS anywhere in the province, to assist them to transition into adult life utilizing various services and programs. Through an individual goal setting “Rites of Passage” plan, the Aboriginal youth directs their journey with the added support of the Worker to hopruflly ensure their success.
Areas such as post secondary education, employment, housing, relationships, financial literacy and so on are typically all part of the planning for example. Referrals come through FACs Niagara, other CAS’s, School Boards, Legal System, Probation , other area agencies, peer and self referral

Youth aged 16 - 24 years typically. Some exceptions can be made on a case by case basis. Educators, other agency personnel are welcome to request an information session to find out more.
